An authorization letter is used to grant authority along with responsibility to someone on behalf of the writer of the letter. They cover a wide variety of intentions and responsibilities. These letters are used for medical, legal, business and personal reasons. Writing an authorization letter for granting approval, delegating responsibility, or giving permission to do almost anything as long as it is not prohibited by law.
Specifically in such letters, it is compulsory to use correct grammar and exact words to communicate your intentions and expectations clearly. Writing authorization letter is not difficult, if it is written in a clear format and language. Be very careful to void further disputes, controversies and complications. Make your authorization letter as professional as possible. The intention of the letter should be clear and expressed thoroughly in the letter. The letter should not give rise to slightest of misunderstanding.
Below is an example of a release of medical records:
I, (Your Name), hereby authorize (Hospital Name) to release to (Name of Person or Doctor with his qualification), any information in my personal medical records, including all x-rays, CAT scans, and any other information pertinent to my treatment while I am under the care of (Hospital Name) during the period from (Date of admission till date of discharge). I give my permission for this medical information to be used for the following purpose: to assist in the diagnosis and treatment of my re-occurring abdominal pain. I do not, however, give permission for any other use or for any re-disclosure of this information.
This is a very basic example, however it would not cover every aspect of a letter of authorisation for medical release records. It is best to get one prepared by a lawyer or attorney for such matters.
